EquiCare - Fixing the front door to Adult Social Care

In England, when adults face challenges with daily life - due to age, illness, or disability - they have a legal right to support from their local council. That support begins with a Care Act Assessment (mandated by the Care Act 2014).

But across West Yorkshire, the assessment system is struggling.
